Course Overview
Essential training
Hour 1: Recordkeeping - Do You Know Where Your Controlled Drugs Are?
RACE TRACKING NUMBER: 20-1322554
Learning Objectives
- Explain why controlled-substance recordkeeping must be a priority in veterinary practice.
- Identify recordkeeping strategies suitable for your practice setting.
- Compare pros and cons of electronic versus paper drug logs.
- Describe how to audit controlled drug records to ensure accuracy and completeness.
Description
Beyond regulatory mandates, effective recordkeeping safeguards both practice integrity and clinical safety. This session provides a structured overview of federal requirements—from DEA Forms 222, 41, and 106 to routine waste and pickup logs—paired with practical strategies to manage and audit records systematically.
Regulatory & closed-system context:
Controlled Substances Act mandates a closed-system lifecycle, requiring practices to track drugs from receipt through administration or disposal.
Required records:
Must include Forms 222 (ordering CII), 41 (disposal), 106 (loss/theft), waste logs, client pickup logs.
Paper vs. electronic logs:
- Paper: intuitive, writable annotations; risks include misplacement and illegible entries.
- Electronic/dispensing cabinet: legible, time-stamped, access-controlled; requires infrastructure.
Inventory & reconciliation:
Treat inventory like balancing a checkbook—beginning stock + receipts – disposals = expected on-hand. Regular audit reconciliations are essential.
Standard operating procedures:
Document SOPs delineating roles, access permissions, ordering, storage, audits, and incident response. Establish cycle-count routines—weekly or shift-based—to identify discrepancies early and document corrective action or escalate unresolved to DEA Form 106.
Key Take-Home Points
- Every controlled-substance transaction must be traceable, with matching documentation.
- Reconcile inventory often; logs must reflect real-time status.
- Choose log systems based on operational context; each has distinct benefits and limitations.
- SOPs must be written, implemented, and auditable.
- Promptly report unresolved discrepancies using DEA Form 106.
Hour 2: Preventing Client Drug Diversion
A proactive approach to protecting your practice
Learning Objectives
- Summarize red flags for recognizing drug-seeking client behavior.
- Describe human-healthcare technologies (PDMPs, e-prescribing) and their limitations in veterinary settings.
- List benefits and legal considerations of in-house dispensing vs. prescribing.
Description
Client diversion poses serious legal and ethical risks in veterinary medicine. This session equips practitioners to detect drug-seeking patterns, understand relevant technologies, and make informed choices between dispensing strategies.
Recognizing diversion:
Watch for frequent refill requests, inconsistent histories, vet-shopping patterns. Maintain logs of near-miss incidents.
Technology in context:
PDMPs and e-prescribing offer control but often are incompatible or inaccessible for veterinary workflows.
Dispensing vs. prescribing:
- In-house dispensing: tight control, immediate access; higher administrative and security burdens.
- Prescribing: less control after issuance; legal responsibility remains with the prescriber. VCPR must be valid.
Operational safeguards:
Restrict access to controlled drugs, train staff, keep detailed client pickup logs including signature and date. Ensure client pickup logs are completed at dispensing. They serve as legal documentation and can clarify misunderstandings if a client denies receipt later.
Key Take-Home Points
- Vigilance for drug-seeking behaviors is essential.
- Human healthcare technologies are useful but may require adaptation for veterinary use.
- Choose dispensing methods strategically, understanding legal accountability remains with the veterinarian.
- Use detailed client pickup logs and control access to minimize diversion risk.
- Train staff and enforce SOPs rigorously; document all unusual incidents.
